Care delivery and data capture in Accountable Care

Some people setting up ACOs have mentioned one critical flaw to the concept: Collecting data for patient visits when they seek care outside of the ACO’s network (on vacation, don’t like in-network specialists, etc). While patients will be given the choice to go wherever they want for care, how can IT staffs help make sure these out of network care episodes get captured and reported for the ACO?

These would most likely be part of the overall process when the patients comes to the office for a visit. Clinical staff may ask the patient if they have been seen outside the network, and if that the case, then the next step is to see if that healthcare organization is participating in the HIE by querying for the patient's info on the HIE or NHIN. This is a very good question and I can only speculate what would happen. We will most likely see different groups handle it differently, but at least this is the best guess.
